All forms are completed through SimplePractice, a safe and secure website.  Once we consent to work together, you will have access to a client portal. All paperwork is completed through the client portal. No need to print, email, mail or save anything. Everything is completed on the website.

• Good Faith Estimate – The Good Faith Estimate protects clients from surprise billing by informing you of the estimated cost of therapy services. The document will be customized to our treatment agreement and provided with your onboarding documents.
